- Duly accomplished BFAR application form.
- Duly accomplished certification that the vessel is not involved in any administrative and judicial cases.
- Duly accomplished affidavit of undertaking.
- Two (2) copies 8“x10″ uncomputerized pictures (port and starboard) showing name of the vessel painted on the hull.
- Grid map indicating the proposed fishing ground
- Original or authenticated copy of the following:
- Certificate of Philippine Registry (CPR)
- Certificate of Ownership (CO)
- Valid/Unexpired Fishing Vessel Safety Certificate (FVSC)
- Authenticated approved articles of incorporation and by-laws for corporation, the primary and secondary purpose is to engage in fishing , or business name registration, certificate for single proprietorship or partnership.
- Fishing logbook for catcher vessel for registration and approval of BFAR upon payment of registration fee of fifty pesos (P50.00).
- Copy of official receipt for payment of CFVGL application, license fees, and cash bond deposit.
- Detailed inspection report.
- Tax identification Number (TIN) issued by BIR.
- Construction clearance or importation clearance (if new vessel).
- Authenticated copy of Deed of Absolute Sale (for change of ownership).
- Duly Accomplished and notarized BFAR application forms (2 copies).
- Drawing of gear design and its specifications, showing the mouth, body, and bunt/bag of the fishing gear.
- Official receipt of CFVGL fee.
- Official receipt of fishing gear registration fee.
